A copy of Birth certificates
Social Security Cards of all members in the household.
Legal paperwork from court is required for other members in the household other than immediate family members.
You will need to sign-in when you drop off your application in order for it to be processed.
Incomplete applications or applications without all requested documents will not be processed.
