On February 18, 2020 we will be accepting applications for people that need special accommodations only from 2:00 pm until 4:00 pm.
On February 19, 2020 others may bring in applications to be checked and processed from 9:00 am to 11:00 am and 2:00 pm to 4:00 pm
Please print off the application on the website https://www.hartsvilleha.org/hcv-forms, fill it out and bring it in along with all the birth certificates and social security cards for all family members in the household.
All Applicants must sign in and let a staff member review their application, additional paperwork may be needed.
If there are household members 18 years of age and older, they will also need to sign additional paperwork.
