Instructions

• Go to www.hartsvilleha.org
• Click on Services & Info
  
• Click on Housing Choice Voucher Program
  
• To the left of the screen click on HCV Forms
  
• Click on full application for Housing Choice Voucher Program
  
• Print off the application form, fill it out completely. You will need birth certificates, social security cards for all household members, (if they are not a family member you will need court documents)
  
• Bring application and documents to the office on the date we will be taking the applications
